Nose ski was dipped down at the front during landing, caught the snow damaging the ski, nose gear, starboard ski and flap. Location of the mishap was Willy Field, Antartica and no injuries resulted with the aircraft returning to flying status shortly after. Date of loss is unknown, reportedly 1992 or 1993.

Crashed near Lagos, Nigeria with all 158 (confirmed) people on board killed. Exact number of those on board is not known as there was no manifest, so was likely more. Aircraft started losing engines one-by-one shortly after take off. The aircraft plowed into the ground in a nose steep angle with only one operating engine three minutes into the flight.
