During a flight from New Zealand it is said two engines seized up causing a divert to Richmond, Australia. On landing the remaining engines failed and the aircraft sat in waiting for a month. It was later determined that the fuel had been contaminated. Date of occurance is unknown.

Written off in ground accident at St. Petersburg.

Ditched off Taiwan while searching for survivors from sunk freighter killing 13 of 14 on board. The only survivor was a PJ. Often mistaken as being with the 57th ARRS, but correct squadron assignment was the 31st ARRS.

Stalled very shortly after take-off at RAF Fairford, England killing all six crew.

Crashed while simulating two engines out at Shiraz, Iran.

Crash on landing at Loc Ninh, South Vietnam.
18 May 1969 [ w/o] 149814 USMC VMGR-152 KC-130F Details
Collided head-on with F-4B 151001 while refuelling another F-4B 151450 and another F-4B (that landed safely) over South Vietnam near Khe Sahn. All six on board the C-130 were killed. The head on crew in the F-4B from VMFA-542 were also killed, while the refueling F-4B from VMFA-314 crew ejected.

Crew chief from the 36th TAS at RAF Mildenhall, United Kingdom started up the aircraft and took off for Langley AFB and crashed into the English Channel.

Crash landed at Ubon RTAFB, Thailand after receiving battle damage over Laos killing two crewmembers.

Burned out during refueling at Islamabad. For many years after, the aircraft's fuselage was in a dump nearby the mishap location.
